Yeah, that can be a headache. I find it is usually an obscure setting like Container (MP4), Multiplexer (Auto), Remuxer (MP4Creator). Sometimes helps to go File -> Load Preset and load one like "AVCHD-to-MP4.xml" that you can copy settings from. Definitely save your settings once you get it working....
